The following questions are being asked of you so that the right puppy and the right placement of each puppy is made by bringing the various requirements of both puppy and his or her new owner into perspective, before a choice of made. I hope you will agree that the animal’s welfare must be my foremost consideration when it comes to placement. Please answer the following questions as completely and honestly as you can, and return this form to the Seller when you have finished. We have the right to deny anyone whom we do not see suitable or fit for one of our dogs or puppies.
